Alexian Brothers Provincial Archives does not collect patient medical records. If you would like to obtain a copy of your medical record, you must contact the hospital directly, even if the hospital is no longer sponsored by Alexian Brothers. Archives does not keep track of subsequent sales or transfers of our former hospitals.
